Waiver of Liability

I represent and warrant that my child/children as listed above are in good health and have no physical conditions,

ailments or disabilities which could endanger my child/children’s health or safety if they were to participate in vigorous

physical activity. For and in consideration of the benefits derived from my participation in the Alameda Waterpolo Club program, I assume all risks and hazards incidental to such participation, including transportation to and from such activities, and do hereby indemnify, release and waive all claims against, Alameda Waterpolo Club, host pools, volunteers, employees and other agents/participants, from all claims of any kind whatsoever, which may arise or hereafter accrue in connection with my participation in the activities of the Alameda Waterpolo Club.

Consent to Participate

As Parent/Guardian of the listed minors, I grant them permission to participate in all activities of the Alameda Waterpolo Club and by my submission of this online registration form to be bound by the terms and conditions of the paragraphs set forth herein.

Agreement to Pay & Termination

I understand and agree to pay for the products and services I purchase. For services that are accompanied with a payment plan or split payments, I assume full responsibility for the entire amount of purchase. I understand that certain water polo sessions are for a multi-week duration and is a set price for this entire duration and I am responsible for this entire amount.  Any unpaid amount will be considered a debt; I will also be responsible for all fees and means necessary associated with collecting the debt.

Termination: The Alameda Water Polo Club reserves the right to terminate at any time whatsoever the services purchased and will grant a full refund for those services.

Code of Conduct

I have read and understand the terms of the Alameda Water Polo Club Code of Conduct for players; parents must agree to abide by these rules and enforce these rules with their minor(s). I further agree to 

        • Parents are encouraged to attend practices and tournaments. However, parents are asked to stay away from the coaches during practices and tournaments. Conversations with coaches should take place either before or after practices and games.
        • Parents should support the athletes on our teams and refrain from making disparaging or negative remarks about or directed at our club or any of our athletes, coaches, opponents, or officials. This includes email, social media, or verbal and written communication.
        • Parents will not communicate the schedules, policies, or make representations in any way on behalf of the Alameda Water Polo Club without the prior written consent of the club.
        • Parents are expected to have rides available for their athletes at the time that practices and games are scheduled, and to get their athletes to all practices and games on time. For tournaments, as a general rule, athletes should arrive at games at least 45minutes prior to the scheduled time of the game. Parents are expected to pick up their athletes immediately following all Club events.

Uniform/Attire/Insurance

All waterpolo players are required to wear approved waterpolo caps at all meets and practices for their own safety. While representing the Alameda Waterpolo Club during games, parents and children shall behave respectfully and in accordance with rules posted at our hosted pools as well as adhering to rules posted at remote locations.

Our league requires all players to be registered with the USA Water Polo silver membership. USA Waterpolo does have a 14 day trial program, however at our sanctioned tournaments and scrimmages, current membership cards are required.

Parent Volunteers

Alameda Waterpolo Club greatly appreciates and accepts assistance from our parents to help run our organization better and to assist at meets and tournaments. Some of the ongoing areas that need assistance are: Meals for players at events, scorekeeping and statistics at tournaments, take pictures at tournaments and send them to our club administrator to upload, and general administrative duties.

Fees and Cancellation

Membership fees (dues) for Alameda Waterpolo Club are not refundable and must be committed to prior to the player entering the water. Dues are to be paid at time of registration; split payments are due each 30 days until completed.

Should a swimmer decide to discontinue participation in the program with Alameda Waterpolo Club, any outstanding fees, including tournament fees, are considered an obligation of the players’ parents, and are payable upon termination of participation.

14 day cancellation policy:  If for any reason your minor does not wish to participate in the Alameda Waterpolo Club within the first 14 days, you are entitled to 100% of your paid dues returned to you.

Medical Release

I further grant permission for appropriate medical treatment to be given to my child/Children in an emergency, and will

be solely responsible for any medical costs.
